Best Shampoo for Thinning Hair (2026): What Works, Ranked by Evidence

Updated 2026-10-2310 min readEvidence-based content

Quick Answer

Nizoral (ketoconazole) is the best shampoo for thinning hair for most people, with the strongest evidence of any shampoo ingredient. Head & Shoulders Classic Clean (1% zinc pyrithione) is the budget pick with a placebo-controlled trial, and piroctone olamine (RULE No.11) has the best evidence in women. No shampoo regrows hair on its own: for pattern thinning, pair one with minoxidil.

"Best shampoo for thinning hair" lists are mostly thickening shampoos chosen for how they make hair look after one wash. That is a real benefit, but it is not the same as slowing hair loss. A few shampoo ingredients have been tested in controlled trials for shedding and hair counts, and they are not the ones on most lists.

We ranked shampoos by that evidence, and by what is actually making your hair thin.

First: Why Is Your Hair Thinning?

The right shampoo depends on the cause, and for most causes the shampoo is a supporting act.

What you noticeLikely causeWhat a shampoo can doWhat actually treats it
Widening part, thinning crown or receding hairline, graduallyPattern hair loss (androgenetic alopecia)A ketoconazole, zinc pyrithione or piroctone shampoo may modestly reduce sheddingMinoxidil, and finasteride for men
Sudden shedding all over, two to three months after an illness, birth, diet or stressTelogen effluviumLittle; it usually resolves on its ownTime, and fixing any deficiency
Flaking, itching, greasy scalp along with sheddingSeborrheic dermatitis or dandruffA medicated shampoo treats the cause directlyKetoconazole, zinc pyrithione, piroctone olamine
Hair that snaps, looks thinner at the ends, but no extra shedding at the rootsBreakage, not hair lossA gentle or thickening shampoo and conditioner helpLess heat, bleach and tight styles
Round bald patchesAlopecia areata or another conditionNothingSee a dermatologist

If you are unsure which applies, or the shedding is heavy, a doctor and a blood test for iron and thyroid problems are a better first step than any shampoo.

The Ingredients With Evidence

Four shampoo actives have been tested for hair, and they work mainly by calming the scalp yeast (Malassezia) and inflammation that make shedding worse.

  • Ketoconazole (Nizoral). The most studied. In a small 1998 study of people with androgenetic alopecia, 2% ketoconazole shampoo improved hair density and the share of growing hairs about as much as 2% minoxidil PMID: 9669136. In men taking finasteride, adding 2% ketoconazole shampoo gave better results than finasteride alone PMID: 12227482. The trials are small and old, and over-the-counter Nizoral is 1%, not the 2% studied.
  • Zinc pyrithione (Head & Shoulders Classic Clean). The best single trial. In a six-month, investigator-blinded study of 200 men with pattern hair loss, a daily 1% zinc pyrithione shampoo increased hair counts compared with a placebo shampoo, by slightly less than half as much as 5% minoxidil PMID: 12932243. The study was run by the shampoo's maker.
  • Piroctone olamine. In an eight-week, double-blind, placebo-controlled study of women with self-perceived thinning, a piroctone olamine shampoo increased hair counts measured by phototrichogram PMID: 34424549. It was also industry-run and short.
  • Caffeine. In a six-month, double-blind trial of women with pattern hair loss, a caffeine-containing shampoo reduced the number of hairs pulled out in a hair-pull test compared with a control shampoo PMID: 29512972. Other caffeine shampoo studies lack a control group PMID: 38764299.

A direct comparison of 1% ketoconazole, 1% piroctone olamine and 1% zinc pyrithione shampoos in 150 men with shedding and dandruff found all three reduced shedding over six months, by 10-17% PMID: 18498517.

Ingredients without evidence for hair growth: biotin, collagen, keratin, argan oil, saw palmetto in a shampoo, and most "growth" botanicals. They can make hair feel nicer, but there are no trials showing they reduce hair loss.

  • Pura D'Or Anti-Thinning Biotin Shampoo, a popular sulfate-free formula that many people like for how their hair feels. Its biotin does nothing for hair loss. See our DHT shampoo rankings for the full entry.
  • Nutrafol Root Purifier Shampoo, a pleasant sulfate-free shampoo whose own clinical claims are about dryness, flakes and oil, not growth. See our Nutrafol shampoo review.

Best Shampoo for Thinning Hair: Women vs Men

Women. Female thinning is often diffuse and can have several causes at once, including pattern hair loss, shedding after a trigger, and iron or thyroid problems. A piroctone olamine shampoo has the best direct trial evidence in women, and ketoconazole is a good choice if the scalp is flaky or oily. Choose a gentle, sulfate-free shampoo for the days in between if your hair is color-treated or dry. If your part is widening, the treatment is minoxidil for women; see female pattern hair loss.

Men. Most thinning in men is male pattern hair loss. Nizoral two or three times a week, with Head & Shoulders Classic Clean or any mild shampoo on other days, is the best-evidenced shampoo routine. It works best as an add-on to minoxidil and finasteride. If you want to block DHT, see our DHT shampoo guide for how much a shampoo can and cannot do.

How to Use a Hair-Loss Shampoo

  • Leave it on. Massage a medicated shampoo into the scalp, not just the lengths, and leave it for three to five minutes before rinsing.
  • Don't overuse ketoconazole. Two or three times a week is enough; daily use can dry the hair. Alternate with a gentle shampoo.
  • Give it time. The trials measured results at two to six months. Judge a shampoo after three months, not three washes.
  • Condition the lengths. Medicated shampoos can be drying. A conditioner on the mid-lengths and ends reduces breakage, which makes hair look thicker.
  • Keep your main treatment. A shampoo is the cheapest, least risky add-on, but the regrowth comes from minoxidil, finasteride or treating the underlying cause.

The Bottom Line

The best shampoo for thinning hair for most people is a ketoconazole shampoo like Nizoral, two or three times a week, because it has the best evidence and costs little. Head & Shoulders Classic Clean is the budget pick with a real trial behind it, and a piroctone olamine shampoo is the best-tested choice for women. Thickening shampoos make fine hair look fuller but do not slow hair loss. Whatever you choose, pair it with a proven treatment if your thinning is patterned.

Frequently Asked Questions

What is the best shampoo for thinning hair?

For most people, a ketoconazole shampoo such as Nizoral, used two or three times a week. Ketoconazole has the most evidence of any shampoo ingredient: in small studies, 2% ketoconazole shampoo improved hair density about as much as 2% minoxidil. Zinc pyrithione (Head & Shoulders Classic Clean) and piroctone olamine also have controlled trials. Caffeine has weaker evidence, and biotin and 'thickening' shampoos have none for hair growth.

Can shampoo really help thinning hair?

A little. A few shampoo actives reduce shedding or modestly increase hair counts, mainly by treating scalp yeast and inflammation. The best result, from a 200-man trial of 1% zinc pyrithione shampoo, was a hair-count gain slightly less than half that of 5% minoxidil. A shampoo is a useful add-on, not a replacement for minoxidil or finasteride.

What is the best shampoo for thinning hair for women?

A piroctone olamine shampoo (such as RULE No.11) has the best direct evidence in women: a placebo-controlled trial in women with thinning hair found more hair after eight weeks. Ketoconazole (Nizoral) is also suitable for women and helps if the scalp is flaky or oily. For female pattern hair loss, minoxidil is the first-line treatment; use the shampoo alongside it.

What is the best shampoo for thinning hair for men?

Nizoral (ketoconazole) two or three times a week, with a gentle shampoo or Head & Shoulders Classic Clean on the other days. For male pattern hair loss, pair it with minoxidil and, if suitable, finasteride; a 2002 study found adding 2% ketoconazole shampoo to finasteride improved results compared with finasteride alone.

Do biotin shampoos work for thinning hair?

No. Biotin does not absorb through the scalp in useful amounts, and even oral biotin only helps hair if you are deficient, which is rare. Biotin shampoos can make hair feel fuller through their other ingredients, but they do not treat hair loss.

Should I avoid sulfates if my hair is thinning?

Sulfates do not cause hair loss. Sulfate-free shampoos are gentler, which helps if your scalp is dry or sensitive or your hair is color-treated, but they do nothing for thinning itself.
